Who processes data and how to contact us

This policy describes data processing when using the FORMED® website and contacting the seller, the sole proprietor ФОП Гришко Антон Олександрович. For personal-data matters, use the published FORMED® contact details or contact us at: 8 Volodymyra Ivasiuka Avenue, building 5, second building level, Kyiv, Ukraine (другий поверх). State in your request that it concerns personal data.

What data is used

When submitting a request is available on the website, the form asks for a name and telephone number for a reply; email and comments are optional. The selected products, quantities, price information, page language, request identifier, time and technical information about the form version and consent given are sent with the request.

This information is needed to respond, clarify requirements and prepare an agreed order. Data about form operation and technical restrictions is also used to protect against abuse. Do not send diagnoses, medical records, patient data, payment-card details or other unnecessary sensitive information through comments.

The relevant lawful grounds for processing apply to steps taken at your request before entering into a contract and to its performance; a separate expression of consent is requested for functions that require it. Your analytics choice does not replace the expression of consent in the enquiry form. Mandatory document retention, where applicable to a particular transaction, is determined by legislation.

Request transmission and third-party services

Submitted requests are processed by the website’s server-side service and stored in an operational D1 log. Resend is used to send a notification to the seller; messages arrive in a Gmail mailbox. Copies of a request may therefore exist in the website log, the sending service and email. Processing by these services is not limited to storage in your browser.

Infrastructure, email, analytics and video providers may process technical information, including network connection data. The use of international services may involve processing outside Ukraine. This policy does not mean that all copies of data are stored only in Ukraine.

Retention periods and storage locations

The application sets a 90-day retention period for request records in the D1 log. Expired records are deleted progressively during subsequent service operation, so this is not a guarantee that all copies are deleted exactly on the ninetieth day. This mechanism does not automatically delete copies in Resend, Gmail or providers’ technical logs. Contact FORMED® to clarify the retention of a particular request or to make a data-related request.

The selected-products list is stored in the browser without contact fields. It can be cleared using website functions or by clearing browser data; no automatic deletion period is set. The sound setting is also stored locally. The comparison and quotation-preparation tools use browser session storage. Data that you enter in the quotation editor may appear in the downloaded document. That document is generated in the browser and is not, by that action alone, sent to the seller.

The form’s contact fields are not written to the local product list. While the tab remains open, data may remain in the form’s memory for a further submission attempt. Your browser’s own autofill functions may operate separately.

Analytics and video

Google Analytics 4 is connected after an affirmative analytics choice. The choice is stored for 180 days; you can opt out using the website’s analytics controls. Analytics is used for page-view events, telephone-link clicks and acceptance of a request by the system. The request’s contact fields are not intended to be transmitted as parameters of those events. Advertising personalisation and Google Signals are disabled in the configuration. This is not a statement that Google does not process any technical data.

YouTube videos are embedded through the youtube-nocookie.com domain. They operate separately from Google Analytics consent: loading the embedded player may establish a connection to YouTube and transmit technical information. Privacy-enhanced mode does not mean that no data is exchanged with the third-party service.

Your rights

You may request access to your personal data, information about its processing, correction, a reasoned objection to processing, or deletion in the cases provided for by law. Consent can be withdrawn; this does not automatically stop processing that has another lawful basis. You have the right to seek protection from the Ukrainian Parliament Commissioner for Human Rights or the courts. Handling a request may require enough information to verify that it concerns your own data; do not send unnecessary documents without a need to do so.
